Fax Cover Sheet Examples

A fax cover sheet is the first page of a fax that lists the sender’s name, phone number, fax number, and instructions for the recipient, along with a subject line and short message describing what’s attached. Faxes are already a fast, reliable way to communicate, but a clear cover sheet matters even more when you’re sending a HIPAA-compliant fax containing protected health information (PHI) — it tells the recipient who sent the document, why, and what to do if it reaches the wrong person. Create a Fax Cover Sheet Template.
Start with a simple template you can reuse for every fax: a header, your contact block, a subject line, and space for a short message. Build it once and save it, and you won't be formatting a cover sheet from scratch every time you send a fax.
Add a Header.
To add a header to your fax cover sheet, simply type the text into the box provided and then click "Add." You can use any font style, size, color, and alignment.
Include Contact Information.
If you're sending a fax to an individual, you should include contact information such as name, address, phone number, email address, and fax number.
Add a Subject Line.
You should also add a subject line to your fax cover sheet. This will help recipients identify the document more easily.
Write a Message.
A message is what you write on the front of the fax cover sheet. It’s usually short and sweet, just enough to let people know who sent the fax and why.
